WHAT YOU NEED TO BRING WITH YOU TO EXPEDITE YOUR VISIT

1. Bring a copy of your insurance card if you wish to bill PEIA insurance or any other insurance.
If not billing insurance, please bring cash or check for $30.00.
2. Print and complete the Flu Clinic Packet to minimize your wait.
NOTE: If you answer “Yes” to any of the allergy questions on the influenza vaccine consent form, then you will be unable to receive the flu vaccine at a campus location. You will be directed to your primary care physician to receive the vaccine.
3. Wear clothing that allows convenient access to the upper arm.
4. Students, please bring your student ID.

INFORMATION FOR EXPECTING MOTHERS

Pregnant women may receive the vaccine (no specific gestation).
Please consult your obstetrician in advance of obtaining the vaccine if you have questions/concerns.

SEEKING A FLU SHOT OUTSIDE OF WVU MEDICINE

Employees who plan to obtain a flu shot at a location other than WVU Medicine are encouraged to contact the provider in advance for information regarding: hours of operation, vaccine availability, insurance coverage, etc. Not all locations accept PEIA insurance, so fees may apply. If an out-of-pocket expense occurs, PEIA-insured employees may seek reimbursement.

HOW TO SEEK REIMBURSEMENT
PEIA insured employees that make a payment at the time of service may seek reimbursement by submitting a HealthSmart Health Benefits Claim Form.
